Nutritional Assessment:

Once your visit has been booked, you will receive 3 forms to fill out and return back to Linda (Client History, Consent form, Blank food record: to keep a 3-day food journal of everything you eat and drink). Many clients will scan their forms back to Linda at eatsmart@lindamccharles.com , but if taking a picture with your phone is easier, simply select large picture and share/email to eatsmart@lindamccharles.com.

Your “Healthy Eating for Healthy Living” virtual journey has been broken down into 2 parts. Part 1 is a 60-minute virtual visit to get to know you. During this time, a brief medical history, social history and nutrition history will be reviewed and completed. At the end of the session, your part 2 virtual visit will be booked. Based on this collection of information, your healthy eating plan will be prepared by Linda and then reviewed in your 30-minute part 2 visit. The cost for the initial assessment (2 parts) and plan preparation is $270.00. Payment is through etransfer or cheque.


Follow up Visits:

It is recommended that your first follow up virtual visit be scheduled within 2 weeks of your plan review. During this 30-minute session your food record will be reviewed and nutrition education will begin. Behaviour modification and ways to overcome challenges will be discussed. It is your responsibility to weigh yourself the morning of your visit and to provide this information to Linda. Goals will be set and nutritional education will be ongoing. Follow up sessions are typically booked every 2 weeks and the cost is $135.00/visit.

Please note that dietary counselling by a Registered Dietitian may be covered under your extended benefits plan. Insurance companies require that the actual dates of service as well as the breakdown of fees be included in your receipt. Your receipt will be provided once payment is received. Most insurance companies will cover up to $500/year.
